The USEA Collegiate Team Application Form is located on the USEA website at:

The designated collegiate representative listed on the application form must be a either a collegiate representative, faculty advisor or coach for the team as well as a current USEA member.
This representative's n will be listed on the USEA website and will act as the liaison between the college and the USEA.
The cost is $75 per year (December 1st - November 30th).
